1)  Min. 2.25 GPA; 2) Must be a graduating Colorado high school senior; 3) Must have received treatment for congenital heart defect(s)/disease or acquired heart-related condition at Children’s Hospital-Colorado or other hospital, OR be a graduating senior from a Colorado high school who is the sibling of someone who has received treatment for congenital heart defect(s)/disease or acquired heart-related condition at Children’s Hospital-Colorado or other hospital; 4) Must plan to attend a college, university, or vocational or trade school upon graduation; 5)
Demonstrated participation in extracurricular and community service activities; 6) Resume of extracurricular and community service activities required; 7) Diagnosis and/or dates of surgeries/medical procedures required.
