1) Must be between the ages of 17 – 25 years of age; 2) Must have been diagnosed with Kidney or Urological disease; 3) Must be enrolled/plan to enroll in an accredited college or university; 4) FAFSA required; 5) Demonstrated participation in extracurricular and community service activities; 6) Tweet required; 7) College or university acceptance letter required; 8) Copy of tuition bill or verifiable information on cost of tuition(and room & board, if applicable); 9) Letters of recommendation from medical professional and from teacher/faculty member must be on official letterhead; 10) Photograph required; 11) Scholarship award payable in the following installments: Up to $2,000.00 per academic year for up to four (4) academic years; 12) Renewable for up to three (3) additional academic years.
