Additional Applicant Requirements:

Must be a graduating high school senior, high school graduate or current college student

Category (Select all that apply):

1) Min. 2.75/4.0 GPA; 2) Must be a member of one (1) of the following categories:

(a) a Purple Heart recipient who is also a member of MOPH, or

(b) his/her spouse, or

(d) the spouse, widow/widower, or direct lineal descendant of a service member killed-in-action or who died of wounds. If the Purple Heart recipient is listed as killed-in-action or died of wounds, supporting documentation must accompany the application;

3) Must be enrolled/enrolling in an accredited U.S. college or university; 4) Demonstrated participation in extracurricular and community service activities; 5) Resume required; 6) Non-refundable $15.00 processing fee required.
