Additional Requirements and Information:

1) Min. 3.0 GPA; 2) Must be currently enrolled in dental school or attending public health residency program; 3) Must be a 2nd, 3rd or 4th year dental student or post-graduate Public Health student; 4) Demonstrated financial need; 5) Must be in good financial standing with educational institution; 6) Proof of good financial standing required; 7) Demonstrated participation in extracurricular and community service activities, particularly as it relates to the National Dental Association and/or the Student National Dental Association (SDNA); 8) One (1) letter of recommendation must be from the school financial officer; 9) One (1) letter of recommendation must be from a faculty member/clinical director; 10) Personal statement required.

NOTE: Applicants should be a member of the SDNA.
