1) Min. 3.0/4.0 GPA; 2) Must be enrolled/enrolling in one of the following accredited graduate program fields:

o Medicine: allopathic or osteopathic

o Dentistry

o Pharmacy

o Nursing (including BSN)

o Public Health/Policy

o Physician Assistant

3) Must be a PREMIER member of the National Hispanic Medical Association; * 4) Demonstrated participation in extracurricular and community service activities; 5) Curriculum Vitae required; 6) Proof of U.S. Citizenship, Residency, or DACA status (e.g., passport, birth certificate, naturalization certificate, driver’s license, or Form I-797C) required; 7) Personal statement required; 8) Transcript(s) required; 9) Unofficial transcript(s) accepted.

NOTE:
